This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions




The Heart Health Program Patient Navigator will serve as first contact and ambassador for the Heart Health Program Team for patients referred for Heart Health Program intervention, specifically cardiac surgical intervention. The navigator will coordinate appropriate consultation, testing, patient and family education, support in decision making, and patient preparation prior to surgery. He/she will follow patients through discharge in collaboration with the care team to help coordinate follow up and home-services based upon their detailed knowledge of the patient and their home situation resulting from their extensive pre-op patient contact. The navigator will work closely with OR booking to determine optimal and equitable case scheduling based on patient preparedness for surgery as well as acuity of patients waiting. The patient navigator will optimize the experience of patients and their families going through Heart Health Program referral in accordance with the philosophy, goals, and missions of Nova Scotia Health Authority.


* Bachelor of Nursing or Bachelor of Science in Nursing
* Registered with the College of Registered Nurses of Nova Scotia (CRNNS)
* 3 – 5 years of nursing experience in Cardiovascular Nursing required
* Experience with patient and stakeholder education
* Experience delivering presentations to large and small groups
* Experience leading and implementing new initiatives
* Experience establishing and measuring outcome indicators
* Demonstrated excellence in interpersonal, communication, and organizational skills with an emphasis on leadership, decision-making, critical thinking skills, motivation, consultative approach and the ability to conceptualize and plan
* Excellent verbal and written communication skills
* Ability to establish and maintain effective working relationships with diverse stakeholder groups
* Strong computer skills with proficiency in MS Office programs and conducting internet research
* Evidence of initiative
* Able to work independently and as part of a team
* Recognizes and prioritizes time sensitive and critical issues and ensures appropriate response.
* Balances competing priorities and mitigates risk in a sensitive environment
* Recognizes and acts on opportunities for improvement

* Competencies in other languages an asset, French preferred
