Job Description

Health Promotion Facilitator

Job Summary:
Work with AHS staff, physicians, and community partners to develop and implement strategies to promote health and prevent disease and injury.
– Participates in the development, implementation and evaluation of a zone Health Promotion, Disease and Injury Prevention strategy to align with the provincial health promotion action plan in areas such as injury prevention (ie. PARTY Program), tobacco reduction, poverty reduction, food security, healthy environments, oral health, etc..
– Provides expertise to assist AHS staff and community partners to coordinate and implement strategies promoting health.
– Plans, develops and facilitates workshops, seminars and presentations related to healthy living for staff and communities.
– Assists communities to be healthier through mentoring, advocacy and education.  Promotes self-responsibility and provides residents with information related to the determinants of health.
– Links individuals and/or communities to relevant health resources within the region or province.
Advocates for healthy public policy related to healthy living within communities in the South Zone
– Participates in the orientation and training of students and staff.

Provide direct support to Community Coalitions and Networks that are working to promote health and address the determinants of health.   
– Identifies, establishes and supports the development of appropriate partnerships and collaborative initiatives to support health promotion goals and objectives.
– Assumes a leadership role in the development and maintenance of strong networks of partners promoting health in areas like injury prevention, tobacco reduction, food security, poverty reduction, health environments, oral health, etc..
– Works with staff in Population Health Promotion, Public Health Nurses, Chronic Disease Prevention and Management, and other AHS departments and community partners to increase opportunities for prevention activities.
– Researches and/or develops resources for use by staff, community partners, and residents in the region.
– Liaison  with other regional, provincial and national agencies, groups and communities involved in health promotion.

Research to assist in the planning, implementing and evaluating of health promotion initiatives    
– Assists in assessing community needs and collecting information related to targets identified in the Health Promotion, Disease and Injury Prevention Action Plan.
– Assists with the development of a reporting process for documenting progress of strategies identified in the Zone Population Health Promotion Workplan and Provincial action plans.
– Reviews health-related data and identifies trends relative to population health and at risk groups.
– Disseminates information on best practice related to health promotion strategies.
– Participates in special studies, evaluation and/or research projects as directed by program manager.
– Identifies potential gaps in services and programs.

Qualifications:
Formal Education
– Degree in Public Health, health promotion preferred.  Degree in social/behavioral sciences, health sciences or related fields will be considered.

Experience
– 1-2 years experience working in a public health/health promotion or community health setting
– 1-2 years experience working in partnership with community groups and agencies preferred
– 1-2  years experience in project/program management preferred
– Tobacco Reduction training (ie. TRAC, Quit Core) and experience in cessation programming is an asset
– Knowledge of injury prevention programs and strategies an asset
– Demonstrated experience in group facilitation required

– Knowledge of health promotion and community development theory and practice
– Excellent evaluation and research skills
– Proven organizational and time management skills
– Demonstrated ability to work independently
– Demonstrated ability to work in a multidisciplinary team
– Computer skills required. Ability to access and proficiently use Microsoft Word, PowerPoint and Excel programs
– Ability to perform duties as outlined on the Physical Demand Statement (PDS) for the position
– Reliable and dependable
– Ability to safeguard AHS information as private and confidential
– Ability to follow and adhere to AHS policies, procedures, goals and objectives.

Other
– Must have valid Alberta Driver’s License and a reliable vehicle.
